Health Net is a Centene subsidiary with Health Net-branded products and provider operations that are specific to the product and market. Its provider site directs practices to a current Network Participation Request page rather than a universal enrollment portal. Confirm the applicable plan, practice location, provider type, and participation form before applying.

Health Net’s California participation forms are expressly limited to physicians, licensed health-care professionals, and ancillary providers with California practice locations. California providers may need different forms for medical, behavioral health, or ancillary participation. Existing providers, delegated medical groups, and provider groups may have different roster or operational paths; an existing portal account does not by itself establish a new network agreement.

Product and service-area availability can change by county and line of business. For Medi-Cal and other managed-care products, verify the current county, delegated entity, and program requirements directly with Health Net and the relevant state program. The enrollment path

Provide the licenses, NPI, liability coverage, and other supporting information requested in the form or during follow-up.
For Medi-Cal or other product participation, confirm the applicable product, county, delegated entity, and any separate enrollment requirements.
Respond to credentialing or contracting follow-up and obtain a written participation decision and effective date.
After contracting, follow Health Net’s provider operations instructions for roster and directory updates.
